Max loss is not guaranteed. It is sized on a pessimistic loss per lot: the stop, plus two spreads allowed beyond it for slippage and a wider spread. In a fast market or a gap a stop can still fill past that, so a loss can be larger. The spread is paid on entry, so a position starts that much behind; its cost is an estimate at the current price.
